I recently bought a 2021 216 and we’re really enjoying it. One issue is that all 4 speakers are in the middle and front. The girls don’t like it very loud so folks in the back can’t hear it while running. Has anyone added speakers in the rear and if so where? I struggling to find a good spot.

You can only add two more speakers if whatever head unit is in there will support it. BUT, you could move the two front speakers to the rear.
Have you tried simply playing with the fade function? It's a very standard setting that pretty much all stereos have... and have had for decades (including cars). Maybe that's all you need to do.

Yeah I balanced to the middle speakers but it’s still not audible when running. Just wish I had some in the rear.

Looking at the pictures on the Grady White website it looks like the middle speakers, especially without having the protection of a cuddy cabin and when cruising, would be difficult to hear as they look like they're essentially inline with where the captain/passengers would be standing at the helm. Does your boat have the T-Top on it? If so, one option might be to get some speakers that hang from the rear of the hardtop facing forward towards the captain/passengers so you can actually hear them while cruising. If not, perhaps a couple speakers just under where you sit in the fiberglass molded seat?

Just to be clear, since we only have what we actually write to convey thoughts... "balance" typically refers to the left/right... but I think you did actually do the fade thing?
But, yeah, stock systems are not the best for trying to listen at speed. To do that, you need to do some mod's - either what I mentioned above (not too hard and inexpensive) or look into adding an amplifier and then more/better speakers.

The setting on the head end is adjust zone volume. I don't know if that helps your issue. The following type of speaker would probably help.
