I used Covers-and-All (check out their website) for some outdoor cushion covers. They did a super job (all were square pieces with piping on the edges) and were pretty reasonable. They do custom work by the inch. I'm going to look at them when I do the interior cushions on my board. If doing this job, I'd let them quote the covers along with others. You can get the foam off the net if replacing. I have no affiliation. Could even use on outside pieces too.
